General instructions for completing the certificates
When completing the certificates, please take into consideration:
- Fill in all fields on the certificate.
- Fill in the fields "Competent local authority issuing this certificate" and "Competent authority of the exporting country" with the name of your municipality's control unit in English.
Ensure that the certificates are numbered with a unique certificate number at the upper right corner of odd-numbered pages. The certificate number should be formatted as XX/YYYY/YYYY, where:
- XX = the name, abbreviation, or number of the supervisory authority or area
- YYYY = the year of issuance of the certificate
- OOOO = a running number specific to the certificate's supervisory unit, ensuring that no two certificates can have the same number.
You can print the completed certificates on standard A4 paper. The municipal official veterinarian will verify the information on the certificates with their signature.
